Client Forms

Preparing for Your First Session with Care and Intention.

A Gentle Beginning

Welcome. I am honoured to walk beside you as you begin this journey toward healing, clarity, and emotional wellness. Whether you are navigating anxiety, grief, trauma, relationship stress, or the challenges of neurodivergence, especially ADHD, therapy is a space where you can feel seen, supported, and safe.

Before we meet, there are two forms I kindly ask you to complete. These are not just paperwork — they are part of how I begin to understand your story, your needs, and how I can best support you. Everything you share is held with care, confidentiality, and respect.

What You'll Need

To help us begin with clarity and trust, please complete the following Therapy Agreement Form.

This form outlines the foundation of our therapeutic relationship, including informed consent, confidentiality, payment for services, and my cancellation and rescheduling policy. It ensures that our work together is grounded in transparency and mutual understanding.

Please bring a signed copy to your first session.

Intake Forms

Depending on the type of support you are seeking, please complete one of the following intake forms.

These forms help me understand your emotional landscape — whether you are struggling with emotional regulation, negative thought patterns, relational tension, or simply feeling stuck. They allow us to begin with intention and compassion.

For sessions involving children, adolescents, or family dynamics

Couples Intake Form

For relationship or couples therapy
